To protect your privacy and give you my full attention, I ask that all clinical talk happens through the Elation portal. I kindly request that you do not send medical questions via a regular email or text.
Urgent Care
If you have an urgent medical need, please call me and leave a voicemail. I monitor these messages personally so I can get back to you as soon as possible. I ask that all patients use their judgement in seeking urgent and emergent care if I am not available.
When You Arrive
Please text me from your car when you arrive and wait for me to confirm before you head up. To ensure the privacy of everyone I see, I ask that you do not wait in the hallway by my office door.
My Billing Plan
Your membership covers one visit each month. If we need extra visits or deep portal work for new issues, I may bill those as e-visits to reflect the additional time I spend on your care.
I will apply a $10 fee to your account if a monthly payment is declined more than once in a single year.
Your Annual Membership
I include one visit per year in your membership. To make sure I can provide you with dedicated time, I will bill any extra appointments beyond that as individual sessions when we meet.
I offer a monthly membership discount for families. Please keep in mind that I can only apply this to your recurring membership fee and not to any individual appointment costs we may have.
Portal Use & E-Visits
I welcome your short questions, but if we need to manage a complex new issue or engage in long portal discussions, I will bill this as an e-visit to account for the specialized care I am providing.
Processing Payment
I process payments monthly to keep our direct care relationship simple. If a payment is repeatedly declined, you will incur a $10 fee for each subsequent declined payment as these re-billing is a time consuming manual process.
My Consultation Path
You may choose to pay per consultation rather than joining as a member. If you choose this, I require you to have a separate primary care doctor to handle your routine and daily medical needs.
If you are not a member, I am unable to offer urgent care or portal messaging between our sessions. I will focus our work entirely on our scheduled time together during your appointments.
